P0401 — EGR Flow Insufficient
Advanced code-specific diagnostic direction. Confirm the fault with scan data, circuit testing and physical inspection before replacing components.
Safety / setup
Work on level ground, secure the vehicle, keep hands clear of belts/fans and disconnect battery power only when the procedure allows it. Record active, pending, history codes and freeze-frame before clearing anything.
Possible causes
- Blocked EGR passages, cooler or intake carbon.
- Failed EGR valve/actuator or position sensor.
- DPFE/differential pressure hose or sensor fault on applicable systems.
- Vacuum supply/control solenoid issue.
- Exhaust restriction or DPF problem changing flow.
Known-fault bulletin direction
Mitsubishi, Isuzu, Ford/Mazda diesels and Toyota diesels commonly suffer EGR/intake carbon or pressure hose issues. Confirm flow change before replacing EGR.
Confirm VIN, engine code, build date and OEM service information before quoting a known pattern failure.
Advanced test path
- Command EGR and watch MAP/MAF change. No change suggests no flow or bad feedback.
- Inspect/clean EGR passages where accessible.
- Check EGR position feedback versus command.
- Inspect DPFE/pressure hoses for split, soot blockage or water.
- Check DPF differential pressure and exhaust temp data on diesels.
Repair order
- Clean/repair EGR passages or cooler restriction.
- Replace proven failed EGR actuator/position sensor.
- Repair pressure hose/sensor fault.
- Address DPF/exhaust restriction if it caused the flow fault.
